Dr. Elseweifi verlängert die durch Zirkumzision kurz gewordene Vorhaut mit Zickzack-Schnitten. Das verschafft eine lange Vorhaut, anders gesagt eine Vorhauthaube, um die Eichel herum. Diese ist meistens unvollständig. Die Ecken der Schnitte werden vernäht. Die Symmetrie wird gewahrt, um ästhetisch zufriedenstellende Ergebnisse zu erzielen. Das Penissegel (Frenulum) wird nicht berührt. Das Ergebnis ist sehr individualisiert.
